Step One
First, we will need to know if you are the patient, or are an advocate for the patient.
If you are the patient, please select ‘Patient’
If you are an authorize representative of the patient, please select ‘Advocate’
Step Two
Next, you will be asked to provide the Campaign Code, located on the patient’s Re-Enrollment letter. If you are unable to locate the Campaign Code, you may locate the Re-Enrollment offer by selecting the diagnosis and providing the patient’s last name.
Step Three
After the Re-Enrollment offer is located, you may Re-Enroll or decline assistance. If multiple programs exist, un-checking a program will indicate to Good Days that you are no longer in need of assistance for that program.
Step Four
Once the appropriate program is selected, you’ll be asked to verify the patient’s annual household income and household size.
Step Five
Then, verify the patient’s primary and secondary insurance. If this information has not changed, you may simply click ‘Next’.
Step Six
If the insurance information should be updated, click the box in the left hand corner to update the insurance information below. Once updated, you may then click NEXT to move onto the following page.
You will then be asked to review the information that will be submitted to Good Days for Re-Enrollment. If the information is correct, click NEXT to continue or use the BACK button to correct any mistakes.
Step Seven
Finally, accept the Applicant Declaration Terms of Consent by selecting “I accept these terms” in the bottom left hand corner of each page. Please type your full name as the signature for this attestation. If you are the patient’s advocate, please make sure to use your full name, not the name of the patient.
Results
Upon submission of the Re-Enrollment information, you will receive an immediate determination of the patient’s Re-Enrollment status. In the top corner, you will see a button which allows you to download the results as a PDF. We recommend that you download the Re-Enrollment results for your records.
